Key Responsibilities

  • •Round and manage clinical care for the assigned SNF population.
  • •Round daily, prescribe medications and treatments, and deliver comprehensive primary care services.
  • •Provide phone or secure messaging consultations to facility nursing staff.
  • •Complete timely documentation for all patient care/encounters in ChartEasy™.
  • •Be on-site and available during in-person scheduled shifts (6-hour shifts between 7AM–4PM).
