Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals into the program via phone, text, and email, and in-person at clinic/community partner locations.
  • •Listen to eligible members, meet them where they are, and conduct multiple outreach attempts as needed.
  • •Educate partners about Pair Team services and link eligible members to the Enhanced Care Management program.
  • •Explain Enhanced Care Management services and benefits, addressing member hesitancy and mistrust.
  • •Manage the end-to-end enrollment funnel, meet enrollment metrics, and provide consistent updates on funnel stages, delays, and blockers.

Key Requirements

  • •Physically located in the United States and able to work PST hours with a quiet, HIPAA-compliant, internet-connected space.
  • •1+ year of experience in a metric or quota-driven environment.
  • •Bilingual in English/Spanish with strong reading, writing, speaking, and listening skills.
  • •Strong technical skills and comfort using multiple technology platforms for virtual ops and internal/external tools.
