Key Responsibilities

  • •Develop, assess, and facilitate complex care management for primarily mental and behavioral health needs, including personalized care plans.
  • •Evaluate member needs via phone and/or in-home visits and recommend/facilitate care/service plans for best outcomes.
  • •Perform telephonic, digital, and/or site visits outreach to assess needs and collaborate with resources.
  • •Coordinate between members/families, community resources, and care providers to ensure services are accessible.
  • •Monitor care plans and member status/outcomes and document care management activities for compliance with applicable regulators.

Key Requirements

  • •Requires a Master’s degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing.
  • •2–4 years of related experience.
  • •For Illinois YouthCare plan: graduate degree and independently licensed behavioral health clinician (e.g., LCSW, LCPC, PsyD) in Illinois, or Bachelor’s Degree and IL RN licensure.
  • •Must reside in Illinois.
  • •Knowledge of the Child Welfare System or DCFS is helpful.
