  • •Deliver compassionate care for patients from age 0–100, including complex chronic conditions such as diabetes, hypertension, obesity, and mental health.
  • •Provide culturally inclusive care and work with a diverse patient population.
  • •Travel monthly to provide in-person care at employer onsite health clinics in Bastrop and McGregor for 2–3 consecutive days.
  • •Use virtual follow-up visits to support patients and coordinate care with the virtual national network.
