Key Responsibilities

  • •Develop and coordinate holistic LTSS care management activities, including creating and maintaining personalized service care plans.
  • •Evaluate member needs and available resources, recommend and/or facilitate care plans to achieve quality, cost-effective outcomes.
  • •Coordinate between members/families/caregivers and care provider teams to ensure identified services are accessible.
  • •Monitor care plans and member status/outcomes, document care management activities for compliance, and provide recommendations based on member needs.
