Nurse Practitioner (Remote, PA License Required)

Provide phone and video-based follow-up care for patients after hospital discharge as part of the Transitions of Care program. Conduct telephonic outreach and scheduled visits focused on medication reconciliation, symptom screening, and ordering needed follow-up labs, medications, or supplies. Build trust with patients, coordinate care across hospital/ED, pharmacies, and community providers, and collaborate with the nursing team to support population health goals.
