Key Responsibilities

  • •Develops and coordinates care management activities and personalized care/service plans based on member needs.
  • •Identifies providers, specialists, and community resources needed for care and facilitates access.
  • •Provides psychosocial and resource support to members/caregivers to address needs such as employment, education, housing, food, and independent living.
  • •Coordinates between members/families/caregivers and care providers to ensure services are accessible in a timely manner.
  • •Collects, documents, and maintains member information and care management activities to ensure compliance with applicable regulators.

Key Requirements

  • •Bachelor’s degree and 2–4 years of related experience.
  • •Graduate from an accredited School of Nursing if holding clinical licensure.
  • •For the Mississippi Magnolia Health plan: bachelor’s or master’s degree in a healthcare-related field (social work, sociology, psychology, public health) and 2–4 years of related experience.
  • •LSW or LMSW preferred for the Mississippi Magnolia Health plan.
  • •Equivalent experience may be accepted based on applicable accomplishments reflecting the level of the position.
