Key Responsibilities

  • •Navigate schedules and partner with the in-home clinical team members
  • •Conduct thorough wellness exams including health risk assessments, medication reviews, and cognitive screenings
  • •Coordinate patient care by evaluating vital signs and arranging blood draws with the in-home team

Key Requirements
