•Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.

Key Requirements

•Physicians (MD/DO) must hold an active U.S. medical license and be board-certified or board-eligible in an appropriate specialty; new graduates who are board-eligible and pursuing licensing are encouraged to apply.
•Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine & Rehabilitation, and Geriatric Medicine.
•Active board certification or board eligibility and compliance with licensure requirements are required.
•Ability to perform essential functions of the position satisfactorily, with or without reasonable accommodation.
•Experience in post-acute care and SNF settings is preferred.
