Provide whole-person care through case management for individuals with serious mental illness, substance use disorder, homelessness, and high medical needs. Maintain a caseload using evidence-based, relationship-driven engagement, conduct telephonic and SMS outreach, and help members set goals and complete Health Action Plans/Shared Care Plans. Coordinate referrals and community support connections, collaborate in case reviews with the care team, and meet encounter metrics while working primarily from home with occasional on-site visits.

•Work with members to identify health and wellness goals and incorporate them into Health Action Plans/Shared Care Plans.
•Coordinate referrals and appointments with community supports and collaborate with the enhanced care management team through case reviews before clinical actions.
