LTSS Service Care Manager

Coordinate holistic long-term care services by assessing member needs, recommending and facilitating individualized service care plans, and connecting members and caregivers with providers and community resources. Monitor care plans and outcomes, perform site visits as needed, and provide education on services and benefits. Maintain compliant documentation across state, federal, and payer requirements, while partnering with long-term care healthcare teams to improve quality and cost-effective care.
