Key Responsibilities

  • •Develop, assess, and coordinate holistic care management activities for long-term care members.
  • •Develop or assist with individualized service care plans/service plans, and identify needed providers/specialists/community resources.
  • •Coordinate between members and caregivers and the care provider team to ensure services are accessible.
  • •Monitor member status and outcomes, and provide recommendations to improve care plans based on member needs.
  • •Collect, document, and maintain long-term care member information and care management activities to ensure compliance with regulators; may perform home and/or site visits.

Key Requirements

  • •RN or LCSW required.
  • •Bachelor's degree and 2–4 years of related experience.

  • •Experience locating community resources, collaborating with caregivers/state agency representatives/providers, and practicing cultural competency.
