Key Responsibilities

  • •Develop, assess, and coordinate LTSS care management activities for members to achieve quality, cost-effective healthcare outcomes.
  • •Develop and maintain personalized service care plans/service plans, evaluating needs and recommending/facilitating best outcomes.
  • •Identify and coordinate providers, specialists, and community resources needed for long-term care plans.
  • •Coordinate between members (and/or caregivers) and the care provider team to ensure services are accessible.
  • •Collect, document, and maintain member information and care management activities to ensure compliance; perform on-site visits as needed and provide education to members and caregivers.

Key Requirements

  • •Bachelor’s degree and 1 year of related experience.
  • •Assess member needs and coordinate holistic care management activities to support quality, cost-effective outcomes.
  • •For Illinois Plan Only: RN licensed in Illinois, or a Bachelor/Master’s degree in an approved human services related field.
  • •For Illinois Plan Only: LPN with one (1) year experience conducting comprehensive assessments and providing formal service for the elderly.
  • •Preference for applicants with past case management, advocacy, or home visits/community travel experience in southern Illinois counties (Johnson or Union).
