Key Requirements

  • •Hold an MD or DO degree and be board-certified in Internal Medicine, Family Medicine, or Geriatric Medicine.
  • •Minimum five years of advanced clinical practice experience, with expertise in chronic disease management and care for vulnerable seniors.
  • •Be (or be willing to become) licensed in Georgia, including DEA and CDS registrations (company support available).
  • •Prior experience in home care (preferred).
