Key Responsibilities

  • •Review your case load each day and prepare technology and tools to service patients.
  • •Navigate your schedule using modern technology and coordinate with in-home clinical team members.
  • •Conduct virtual wellness visits, including thorough health risk assessments, medication reviews, and cognitive screenings.
  • •Work directly with in-home team members to evaluate vital signs, arrange blood draws, and carry out patient needs.
  • •Provide care coordination and management, including preventive care interventions, medication management, referrals, and documentation.

Key Requirements

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner license in Louisiana.
  • •Consistently demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion to provide high-quality health services.
  • •Ability to use technology for charting and HIPAA secure messaging apps for care coordination.
  • •Pass national background check and complete valid clinical license search.
