Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals virtually and in-person at partner locations.

  • •Explain Enhanced Care Management services and benefits, addressing initial hesitancy and distrust.
  • •Manage the end-to-end enrollment funnel, including tracking stages, delays, and blockers, and meeting enrollment metrics.
