Key Responsibilities

  • •Provide administrative support for the Foster Care/DCYF team, assisting Care Managers and Care Navigators with tasks such as emailing and updating monitoring care plans.
  • •Maintain assigned monitoring care plans and complete member outreach to support comprehensive assessments as needed.
  • •Coordinate care activities based on care plans/services plans, working with healthcare and community providers and members/caregivers to accommodate changes.
  • •Serve as a liaison for member and provider inquiries related to care plans/services plans and facilitate continuity of care.
  • •Document and maintain member records in accordance with state and regulatory requirements and support complex or escalated issues, including referrals to Care Manager and community organizations.
