Services Coordinator

Coordinate developmental assessment appointments and therapy services for children with developmental delays, disabilities, or special needs. Serve as the primary liaison between families, healthcare providers, therapists, schools, and community agencies. Conduct intake interviews, gather relevant information, and develop and monitor individualized service plans while scheduling services and maintaining confidential records. Ensure timely referrals, follow up on missed appointments, educate families on available resources, and support compliance and quality improvement.
