Key Requirements

  • •Must be located in Sacramento County and meet California CHW minimum qualifications via the certificate pathway or 2,000+ hours of CHW work in the prior three years.
  • •Reliable transportation and a flexible schedule for face-to-face outreach.
  • •Strong written and verbal communication skills, comfortable engaging with staff, community members, families, and partners in person, by video, and by phone.
  • •Strong computer skills and ability to learn and operate web-based systems and state-required programs.
  • •Maintain confidentiality and professionalism by following HIPPA/health information confidentiality guidelines.
