Clinical Pharmacist-Inpatient

Provide inpatient clinical pharmacy services by reviewing medication therapy for appropriateness, dosing, interactions, contraindications, and monitoring needs. Participate in multidisciplinary ward rounds with evidence-based recommendations, monitor responses and adverse drug reactions, and support antimicrobial stewardship. Document clinical interventions and medication incidents, counsel patients and families, educate healthcare professionals, and contribute to medication-use evaluations, clinical guidelines, formulary reviews, and quality improvement initiatives while maintaining compliance with hospital policies.
