Bilingual Field Care Coordinator / Field Case Management

Coordinate whole-person care for Medicaid and Medicare members by conducting in-person, field-based visits for onboarding, health readings, and follow-up. Serve as the primary point of contact for members with complex medical, behavioral, and social needs, coordinating appointments and referrals across care resources. Maintain accurate HIPAA-compliant documentation, build trust with diverse populations, and collaborate with nurses and behavioral health care managers to improve adherence and care continuity.
