Key Responsibilities

  • •Serve as the primary point of contact for members with complex medical, behavioral, and social needs
  • •Complete new patient onboardings and deliver care packages or collect health readings during in-person visits
  • •Conduct in-person assessments, provide ongoing support, and encourage adherence to care plans
  • •Coordinate services, appointments, and referrals across primary care, behavioral health, and social support resources
  • •Maintain accurate documentation and collaborate with the internal care team to support care continuity
