Clinical Social Worker

Assist clinical social work services by supporting psychosocial follow-up for patients and families. Conduct preliminary client interviews and gather social/demographic information under supervision. Help patients complete referral and service paperwork, coordinate community resources and agency contacts, and maintain accurate case files and documentation. Facilitate communication between patients, families, clinical social workers, and hospital staff while providing general emotional support within permitted scope.
