Care Manager (RN)

Develop and manage complex care plans for members with primarily physical health needs, assessing barriers, social determinants of health, and care priorities. Coordinate with providers and community resources to address unmet needs, provide ongoing follow-up and monitoring, and facilitate timely access to care. Document and maintain member information to meet regulatory requirements, and educate members and families on disease processes, care gaps, and available healthcare benefits.
