Key Responsibilities

  • •Develop, assess, and facilitate complex care management activities, primarily for mental and behavioral health needs members.
  • •Evaluate member needs via phone or in-home visits and recommend and/or facilitate care plans and service plans.
  • •Develop ongoing care plans for high-acuity members and identify needed providers, specialists, and community resources.
  • •Coordinate between members (and caregivers), community resources, and care provider teams to ensure timely access to services.
  • •Monitor care plans and member status/outcomes, provide education to members and families, and document activities for regulatory compliance.

Key Requirements
