Job Summary

Support community connection activities by connecting members with known community and care resources to advance care management. Coordinate community outreach, respond to community and care inquiries, and document resources in line with health plan standards. Partner with clinical nurses and social workers by providing outreach, education, and member support, including non-clinical and non-medical assessments to refer members to appropriate services. Perform telephonic/in-person outreach and possible home visits.

•Conduct telephonic/in-person outreach and non-clinical/non-medical assessments (including possible home visits) to refer members to appropriate care/services.

Key Requirements

•High school diploma or GED.
•1–2 years of related experience.
•Working knowledge of Social Determinants of Health (SDOH) barriers.
•Ability to conduct non-clinical general health assessments and non-medical assessments (e.g., home safety, transportation, employment) to refer members to appropriate services.
•Ability to provide member support through outreach, education, informal guidance, and correspondence/educational materials.
