Job Summary

Assist in assessing and coordinating holistic long-term services and support (LTSS) care management for Physical Disability and Aging Waiver members. Develop and monitor personalized service care plans, connect members with providers and community resources, and educate members and families/caregivers on available services and benefits. Maintain compliant documentation, collaborate with care teams and partners, and perform on-site visits as needed.

•Evaluate members’ needs and available resources, then recommend and/or facilitate the best plan of outcome.
•Develop and maintain long-term care service care plans, identifying required providers, specialists, and community resources.
•Coordinate between members/families/caregivers and the care provider team to ensure services are accessible.
•Provide resource support and education on procedures, provider instructions, service options, referrals, and healthcare benefits.
•Collect, document, and maintain member information and care management activities to meet state, federal, and payer regulatory requirements, including on-site visits as needed.

Key Requirements

•Bachelor’s degree and 1 year of related experience.
•Experience with waiver, case management, advocacy, or home visits/community travel (preferred per posting).
•For the Illinois Physically Disabled/Elderly population: RN licensed in Illinois.
•For the Illinois Physically Disabled/Elderly population: a human services-related bachelor’s or master’s degree (as defined in posting).
•For the Illinois Physically Disabled/Elderly population: LPN with 1 year of experience conducting comprehensive assessments for the elderly.
