Key Responsibilities

  • •Provide virtual care to Medicare patients with chronic, complex, and overlapping medical, behavioral health, and social needs.
  • •Supervise nurse practitioners delivering multidisciplinary care for patients with serious mental illness/substance use disorder, experiencing homelessness, and/or high medical needs.
  • •Collaborate with physicians, nurse practitioners, care managers, and the broader care team to deliver thoughtful, high-quality clinical care.
  • •Support care coordination and use internal care delivery technology to enable best-in-class virtual healthcare.
  • •Help develop and refine clinical programs to address patient barriers and gaps holistically.

Salary: USD 125 hourly

Key Requirements

  • •2+ years of clinical experience serving patients with complex social and healthcare needs, with passion for equitable care.
  • •Board certification or eligibility as a Family Physician; eligible to enroll in Medicare.
  • •Experience with multidisciplinary teams and digital-first healthcare (e.g., telemedicine) across tech platforms.
  • •Medial license in California (plus additional state licensure or willingness to obtain additional licenses) and active CA DEA or willingness to obtain one.
  • •Physically located in California with a quiet, HIPAA-compliant, internet-connected work space and ability to work PST hours for urgent needs/support.
