Attending Physician

Serve as the attending physician for patients in each assigned SNF, collaborating with NPs/PAs to manage daily patient care and provide high-quality, patient-centered primary care. In this role you will diagnose and treat acute and chronic conditions and participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives, leveraging flexible scheduling and advanced clinical technology to deliver care in the post-acute setting.
