Partner with Sprinter in-home team members to evaluate vital signs, coordinate blood draws, and support patient-specific needs. Handle comprehensive care coordination and documentation, including preventive interventions, referrals, and community resource support while maintaining high-quality, HIPAA-compliant communication.
