Key Responsibilities

  • •Review case load and prepare technology to support patient visits.
  • •Conduct virtual adult and/or pediatric wellness visits and health assessments.
  • •Perform thorough wellness exams including health risk assessments, medication reviews, and cognitive screenings.
  • •Work directly with in-home team members (Sprinters) to coordinate vital sign evaluation and blood draws.
  • •Provide care coordination and management, including preventive interventions, referrals, community resources, and documentation of findings.

Key Requirements

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner license in Louisiana.
  • •Strong written and verbal communication skills.
  • •Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps.
  • •Pass national background check and complete valid clinical license search.
