Patient Care Coordinator (WFH)

Coordinate patient and family support by acting as the central liaison to internal and external services. Conduct initial assessments, develop and manage individualized support plans, and connect families to healthcare providers and social service resources. Maintain confidential records and monitor members’ well-being while providing ongoing guidance. Communicate across stakeholders to deliver a seamless, supportive experience aligned to youth well-being and future planning.
