Key Responsibilities

  • •Supervise an interdisciplinary team and facilitate daily care coordination meetings to guide treatment plans and improve patient outcomes.
  • •Lead the intake process by connecting new patients to needed clinical resources and ensuring individualized treatment plans are developed.
  • •Direct care for complex patients, offering support to the clinical team and intervening as needed with empathy and expertise.
  • •Partner with clinical and operational stakeholders to devise and implement strategic initiatives improving quality, member outcomes, and cost-efficiency.
  • •Proactively identify issues and drive continuous improvement by documenting lessons learned; complete required training to stay aligned with best practices.

Key Requirements

  • •Hold an unrestricted registered license as a Psychologist, LCSW, LMSW, LMFT, or LPC in California.
  • •Demonstrated experience working with severe mental illness (SMI), other mental health challenges, and addiction.
  • •Experience counseling patients and creating personalized, individualized treatment plans.
  • •Ability to lead, coach, and negotiate to inspire a clinical team and support both patients and the organization.
  • •Strong communication and interpersonal skills for one-on-one and group settings, with strong organizational skills and basic computer proficiency.
