Community Resource Coordinator II

Support community connection for care management by helping members access known community and care resources in a cost-effective way. Coordinate outreach and provide health education and member support by handling inquiries, correspondence, and educational materials. Conduct non-clinical general health and non-medical assessments (including home safety and transportation/employment needs) and perform telephonic and in-person outreach to locate hard-to-reach individuals and families, documenting resources and assisting nurses and social workers.
