Key Responsibilities

  • •Coordinate care management activities to ensure services are delivered and continuity of care/member satisfaction is achieved.
  • •Perform member outreach via telephone or home visits, discussing the care plan/service plan, next steps, resources, questions, concerns, and education as appropriate.
  • •Coordinate care activities based on the care plan/service plan and work with healthcare/community providers and members/caregivers to accommodate changes.
  • •Support member and provider inquiries related to care plan/service plan and communicate with care managers and practitioners to facilitate services.
  • •Document and maintain member records per state and regulatory requirements and distribute information to providers as needed.

Key Requirements

  • •High school diploma or GED.
  • •1–2 years of related experience.
  • •For Arkansas Total Care plan: bachelor’s degree in social science/health-related field, or high school diploma with at least 1 year of experience coordinating care for developmentally/intellectually disabled or behavioral health clients.
  • •Designated as safety sensitive in Arkansas; requires a driver’s license, child and adult maltreatment check (before hire and recurring), and a drug screen (at time of hire and recurring).
  • •Must reside in AR or a border city; field-based role supporting members in Pope and Johnson counties (additional travel may be required).
