Remote Part-Time Nurse Practitioner (1099)

Provide patient-centered, preventive care to Clover Medicare Advantage members via remote visits. Conduct comprehensive health assessments, review histories and psychosocial status, document encounter notes in Clover Assistant and EMR, and help at-risk members transition home after facilities stay. Create personalized care plans with primary care physicians, close care gaps (including labs and prescriptions), perform point-of-care diagnostics, and coordinate follow-up with interdisciplinary care teams.
