Job Summary

Coordinate whole-person care for individuals with serious mental illness, substance use disorder, homelessness, and high medical needs. Maintain an ongoing caseload using evidence-based approaches, conduct telephonic and SMS outreach, and support members in identifying goals and building Shared Care Plans. Collaborate with the Enhanced Care Management team through case reviews, meet encounter metrics, and help connect members to community supports. Primarily remote with 2–3 on-site community visits per month in California.

•Maintain an ongoing caseload using evidence-based approaches to promote engagement and achievement of health goals.
•Use relationship-based strategies to support members with social support navigation.
•Conduct periodic telephonic and SMS outreach for timely follow-up.
•Work with members to identify health/wellness goals and incorporate them into a Health Action Plan/Shared Care Plan.
•Collaborate with the Enhanced Care Management team through systematic case reviews and delegated task support before taking clinical actions.

Travel: Low travel

Key Requirements

•1+ years of general work experience (case management preferred).
•Be physically located in California.
•Bilingual in English/Spanish with strong cultural fluency.

•Comfort with technology (CRM databases, Excel, Word, email, video conferencing) and a valid driver’s license with auto liability insurance.
