Key Responsibilities

  • •Evaluate member needs and available resources, recommending and facilitating care plan outcomes.
  • •Develop and maintain long-term care service plans and identify needed providers, specialists, and community resources.
  • •Coordinate between members/caregivers and the care provider team to ensure services are accessible.
  • •Provide resource support and education to members and families/caregivers on procedures, service options, referrals, and healthcare benefits.
  • •Collect, document, and maintain member information and care management activities to ensure compliance; perform on-site visits as appropriate.

Key Requirements

  • •Bachelor's degree and 1 year of related experience.
  • •For Illinois physically disabled/elderly: RN licensed in Illinois.
  • •For Illinois physically disabled/elderly: Bachelor’s or Master’s in a human services related field.
  • •For Illinois physically disabled/elderly: LPN with 1 year experience conducting comprehensive assessments and providing formal service for the elderly.
  • •Preference for past case management, advocacy, or home visits/community travel experience.
