Care Coordinator II

Coordinate and support care management activities for members by working with the senior care management team, healthcare providers, and community partners. Conduct member outreach by phone or home visits, help discuss and track care/service plans, complete assessments and screening for complex needs, and document member records in line with state and regulatory requirements. Triage and escalate complex requests while ensuring continuity of care and member satisfaction.
