Care Manager (LMSW)

Develop and manage complex care for members with primarily mental and behavioral health needs, using assessments through phone and in-home visits. Create and monitor personalized care/service plans, coordinate with providers and community resources, and educate members and families on treatment options and healthcare benefits. Maintain accurate documentation to meet regulatory requirements and provide recommendations to improve care quality and cost-effectiveness, with up to 75% local travel.
