Key Responsibilities

  • •Assess member needs via phone or in-home visits and recommend and/or facilitate care/service plans for best outcomes.
  • •Develop ongoing care plans for members with high acuity and identify required providers, specialists, and community resources.
  • •Coordinate between members/family and the care provider team to ensure timely access to services.
  • •Monitor care plans and member status/outcomes and provide recommendations based on changes in clinical symptoms and treatment side effects/complications.
  • •Collect, document, and maintain member information and care management activities to ensure compliance with applicable regulators.

Key Requirements
