  • •Manage complex and chronic illnesses for Galileo patients through the proprietary app and your clinical judgment.
  • •Deliver compassionate, empathetic care to patients across all ages and needs, supporting shared decision-making.
  • •Collaborate with the clinical care team to solve patient problems efficiently using an agile, can-do approach.
  • •Work evening and weekend coverage in 4-hour shifts, meeting minimum weekly hour requirements.
