Key Responsibilities

  • •Support care management activities and assigned member teams to ensure services are delivered and continuity of care is maintained.
  • •Conduct member outreach by phone and/or home visits, engage members/caregivers, discuss care/service plans, and provide ongoing education as appropriate.
  • •Coordinate care activities based on care/service plans and work with healthcare/community providers, partners, members, and caregivers to accommodate changes.
  • •Serve as support for member and provider inquiries, requests, or concerns related to care/service plans.
  • •Maintain member records in accordance with state and regulatory requirements and make referrals to Care Manager, community-based organizations, and Disease Manager as needed.
