Key Responsibilities

Key Requirements

  • •Certified Nurse Practitioner in Adult, Family or Geriatrics.
  • •Active state licensure in PA, with the ability to obtain additional state licensures.
  • •5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
  • •Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions.
  • •Motivational interviewing skills and comfort in a digital-first, virtual care environment.
