Key Responsibilities

  • •Collaborate with the interdisciplinary care team to share observations and support coordinated member care in team meetings.
  • •Identify member needs and connect members with community resources and supports that promote wellness.

Key Requirements

  • •Detail-oriented, organized, and comfortable supporting members one-on-one or in group settings.
  • •Basic computer skills; able to plan and design events and activities.
