Job Summary

Provide hands-on home care to Clover members as a Certified Medical Assistant within the Clover Care Visit team. Partner with Providers before, during, and after visits to support patient needs, take vitals, assist with translation, perform phlebotomy as directed, and help close care gaps. Maintain accurate documentation and coordinate through EHR/EMR systems while ensuring well-prepared visits and supporting outreach to schedule care.

Key Responsibilities

•Partner with Providers before, during, and after visits to support patient care.
•Be a trusted point of contact for members during visits, helping with translation, gathering information, taking vitals, and answering questions.
•Perform phlebotomy and other essential medical tasks as directed by the Provider.
•Review consent forms, help close care gaps, and keep documentation accurate.
•Coordinate visits and outreach through electronic health record (EHR) systems; drive to and conduct member visits in their homes.

Salary: USD 22.21 - 24.42 hourly

Key Requirements

•Hold a Medical Assistant certification from an accredited program.
•At least 1 year of healthcare experience (home-based care a plus).
•Basic Life Support (BLS) certification (or ability to obtain within 30 days of hire).
•Fluency in both English and Spanish with strong verbal and written communication skills.
•Have a valid driver’s license, reliable transportation, and basic computer proficiency for documentation in electronic medical records.
