Key Responsibilities

  • •Support care management activities by coordinating services to ensure continuity of care and member satisfaction
  • •Perform member outreach telephonically or through home visits and discuss care plan/service plan next steps, resources, questions, and concerns
  • •Make inbound and outbound calls and complete member assessments to identify barriers to care and gather health and social information
  • •Coordinate care activities based on the care plan/service plan and work with healthcare and community providers and partners
  • •Document and maintain member records and assist with service inquiries, requests, or concerns related to the care plan/service plan
