Patient Engagement Specialist I

Serve as the first voice for patients by following up on inbound requests and proactively reaching out through personalized outreach. Use an auto-dialer to make 150–200 outbound calls daily, connect with patients, and book appointments while building trust with patients and caregivers. Educate patients on the benefits of at-home services, understand partner care models, collaborate with healthcare professionals, and escalate concerns to other teams as needed.
