Care Manager RN NICU

Provide complex NICU-focused care management for members, including assessing needs and barriers to care, developing and updating personalized care/service plans, and coordinating with providers and community resources. Monitor member status and care plan progress, perform outreach (telephonic/digital/home or other), document and maintain records for compliance, and educate members and families on disease processes, care options, and next steps.
