LTSS Service Care Coordinator

Coordinate holistic care management for long-term support services (LTSS) members, including evaluating needs, recommending care plan/service plan options, and identifying providers and community resources. Work with members, families/caregivers, and the care provider team to ensure services are accessible, monitor outcomes, provide education, and document activities for compliance. Perform 75% local home visits within the assigned Chicago-area territory while working from home for the remaining time.
