Key Responsibilities

•Conduct virtual evaluations for acute and routine clinical concerns and manage changes in resident condition.
•Provide chronic disease management and follow-up care, including reviewing diagnostic testing and laboratory results.
•Coordinate telemedicine coverage during scheduled daytime hours and maintain availability using approved telehealth platforms.
•Respond to urgent facility requests within established service-level expectations and coordinate daily visit schedules with assigned Clinical Liaisons.
•Complete encounter documentation within required timeframes, ensure billing/coding accuracy, and maintain compliance with telemedicine and patient privacy regulations.

Key Requirements

•Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Florida, with willingness/ability to obtain multi-state licensure.
•Minimum 2 years of experience in post-acute care, long-term care, geriatrics, hospital medicine, or primary care.
•Active licensure in multiple states, or willingness to obtain and maintain multi-state licensure for ongoing coverage.
•Active DEA registration.
•Strong clinical communication skills and comfort working in a telemedicine environment.
