Key Responsibilities

  • •Conduct in-home assessments for Aging and Physical Disabilities Waiver members to evaluate needs and resources.
  • •Develop, coordinate, and monitor personalized long-term care service care plans to improve outcomes.
  • •Coordinate between members, families/caregivers, and the care provider team to ensure identified services are accessible.
  • •Provide education and resource support to members and families/caregivers, including referrals and healthcare benefits guidance.
  • •Collect, document, and maintain member information and care management activities in compliance with state, federal, and payer requirements.

Key Requirements

  • •Bachelor's degree and 1 year of related experience.

  • •For Illinois: Bachelor’s or Master’s degree prepared in a human services related field.
  • •For Illinois: LPN with 1 year experience conducting comprehensive assessments and providing formal service for the elderly.
  • •Ability to conduct in-home assessments, develop care plans, document care management activities, and ensure compliance with applicable regulations.
