Job Summary

Coordinate holistic long-term services and supports (LTSS) for Aging and Physical Disabilities Waiver members by conducting in-home assessments, developing and monitoring personalized care plans, and connecting members with healthcare providers, community resources, and DME services. Support families/caregivers with education and resource navigation, ensure care plans and documentation comply with state, federal, and payer requirements, and provide feedback to improve quality and cost-effective service delivery.

Key Responsibilities

•Conduct in-home assessments for Aging and Physical Disabilities Waiver members to evaluate needs and resources.
•Develop, coordinate, and monitor personalized long-term care service care plans to improve outcomes.
•Coordinate between members, families/caregivers, and the care provider team to ensure identified services are accessible.
•Provide education and resource support to members and families/caregivers, including referrals and healthcare benefits guidance.
•Collect, document, and maintain member information and care management activities in compliance with state, federal, and payer requirements.

•Bachelor's degree and 1 year of related experience.
•For Illinois: RN licensed in Illinois.
•For Illinois: Bachelor’s or Master’s degree prepared in a human services related field.

•Ability to conduct in-home assessments, develop care plans, document care management activities, and ensure compliance with applicable regulations.
