Medical Director and Attending Physician, (Part-Time)

Medical Director and Attending Physician role overseeing assigned skilled nursing facilities, providing patient-centered primary care, and supervising daily care with NPs/PAs. You’ll participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives, all within a flexible, part-time schedule supported by advanced technology and a collaborative clinical team.
